birth details (if requesting a free astrological report membership gift)

within a spreadsheet stored within Aquarius Severn's password-protected Google Drive
within our Jotform password-protected account. The membership form on our website is hosted by Jotform.com which allows us to integrate membership details and payment into a single form.
In addition we may also collect information in order to:
process payments, e.g. for talk, workshop or course bookings
Payment information, e.g. card details are processed securely by our payment provider PayPal
process online votes for committee members at our AGM
Your name and email address is required as proof of membership and stored in our secure SurveyMonkey.com account.
We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, we have put in place suitable physical, electronic and managerial procedures to safeguard and secure the information we collect from you.
We will not sell, distribute or lease your personal information to, or share it with third parties unless we have your permission or are required by law to do so. We will however share relevant information solely to facilitate payments with our payment processor, or with our email newsletter service provider solely in order to facilitate your newsletter subscription.
Controlling your personal information
You can delete or amend your email subscription by clicking the Update Subscription Preferences link at the bottom of any of our newsletters in order to change your name or email address.
You may also request details of personal information which we hold about you under the Data Protection Act 1998 and the General Data Protection Regulations 2018. If you would like a copy of the information please email firstname.lastname@example.org and we will send it to you within one month. Please use the following subject line of your email: Subject Access Request.
If you believe that any information we are holding on you is incorrect or incomplete, let us know and we will promptly correct any information found to be incorrect.
